Italian Cheese Melt Recipe?

I had a sub sandwich a while back from a place called Hopkins Eatery and it was amazing. I am trying to figure out how to make it. It was called the italian cheese melt and it had mushrooms, onions, a bunch of different cheeses, tomato and some other stuff I can’t remember. Has anyone had this before or know how to make it? Thanks

    From the Menu at Hopkins-Italian Cheese Melt
    A scrumptious combination of melted Provolone, Cheddar and Parmesan cheeses, Mushrooms, Onions, Peppers, Tomatoes. Served hot on an Italian bun with Mayonnaise, and Parmesan dressing.
    So I would saute 1 cup mushrooms, 1/2 cup diced onions, 1/2 cup diced peppers ( or buy the pickled italain sweet peppers, those are great..) in 2 tbs olive oil until softened, then add 1 cup diced tomatoes for a few minutes. Spread Mayo and parmesan cheese on an italian bun, top with cheddar, provolone, and parmesan, and broil until cheese is melted. You could dip in a parmesan dressing, -I think hidden valley has a parmesan ranch.
